After Bruno falls from the swing and hurts himself, Pavel tends to the boy's wounds. After this, Pavel "wash[es] his hands carefully, even scrubbing under his nails with a wire brush" before he starts peeling carrots. He is attentive to his work of carrot peeling. The carrots seem to "[take] up so much of his attention." While Pavel works, Bruno questions the man's ability to care for his injuries, telling that him that he is "'not a doctor.'" Pavel admits, with much hesitation, that he used to be a doctor. Bruno finds this impossible, because Pavel's job in his home is to serve dinner and prepare food in the kitchen. Pavel explains that he used to be a doctor before he came to Bruno's home.
Pavel was a doctor, and being one he worked with his hands. As a doctor, he had to pay attention to detail. He peels the carrots carefully with his hands, paying close attention to them. His treatment of the carrots serves as a comparison to how he would treat a patient in his work as a doctor. Bruno notices that Pavel is paying more attention to the carrots than to him because he is only concerned with his own injuries.
